Burial headstone approval

To start the form please confirm that the following statements are true:

• Application is to be filled by funeral director or stonemason.
• Monuments may only be erected with the express authority of the lair holder.
• Maximum dimensions permitted have been restricted for Health and Safety reasons. Works to erect a monument must only be undertaken by a registered monumental mason
• Monuments on lairs for casket only burials, where a headstone is acceptable, will be no higher than 0.76m, and where a tablet is permissible will be no higher than 0.51m. These must not encroach on any other lairs
• Memorial wall plaques will be no larger than the undernoted size:
  • Single lair – 380mm wide x 210mm high x 35mm deep
  • Double lair – 380mm wide x 505mm high x 50mm deep


• Memorial design diagram or drawing should be available to upload with the application.
• If the deceased is under 18, no fee is required.
